Please send this form with your payment of check or money order made payable to Hyde Concepts Website Design. Include one(1) to three(3) photos, no larger than 5" x 7", and be sure that each photo is clearly labeled with the child's name, D.O.B., parents name, and a phone number. Please send copies, as these photos will not be returned.

Contest Rules and Regulations
One baby will be chosen as the Happy Baby Winner every 3 months. The winner will receive a 1 year contract with Atlanta's Young Faces and a $30 gift certificate to Babies R'Us. Pictures will be featured on the website through the current contest period and showcased throughout the time of the following contest. Kids ages 0-3 years of age are welcome to enter. Contest winners will be chosen one(1) month after the close of each contest period. Schedule listed below.

Contest Schedule:
Period One - Jan. 1 through March 31. Winner chosen by April 30th.
Period Two - April 1 through June 30. Winner chosen by July 31.
Period Three - July 1 through Sept. 30. Winner chosen by Oct. 31.
Period Four - Oct. 1st through Dec. 31. Winner chosen by Jan. 31.
